We ask that members of the sorority complete the Visitor Verification Form (accessed below) along with one or more of the forms of membership confirmation listed below:
Forms of Membership Confirmation
- copy of membership card
- copy of membership certificate
- copy of letter of verification
- membership number
Send the completed form to the 1st Vice President, 1stvp@dst-oscalumnae.org 72 hours before the event, with the subject: Visitor Verification Form Submission
The chapter will contact you once verification is complete.
